West Florida AHEC Diabetes Care Clinic

The WFAHEC Diabetes Care Clinic is a clinic concerned with health promotion and prevention for persons with diabetes. Diabetes screening and management is offered to clients of all ages. The clinic, located at the Santa Rosa County Health Department in Milton, is a joint project of West FL AHEC, University of South Alabama College of Nursing and Santa Rosa County Health Department.

This clinic is only open on a certain day of the week and can provide potential clinical training opportunities as arranged under the guidance of Judy Azok, MSN, GNP, ARNP-BC. This is an ideal training experience for nurse practitioner students.

Health Services are provided under the direction of Advanced Practice Nurses with a physician consultant or Advanced Practice graduate students working under the supervision of an Advanced Practice Nurse.
